Microneedling in cosmetics: mechanism, risks and regulatory challenges

Microneedling, also known as collagen induction therapy, is a technique that uses fine needles to create controlled micro-injuries in the skin. These micro-injuries trigger the skin’s natural repair process, stimulating the production of collagen and elastin.

What is microneedling?

In the ever-evolving landscape of aesthetic beauty, few procedures have garnered as much attention as microneedling. Once reserved for clinical dermatological environments, the technology has increasingly trickled down into the consumer beauty market. While microneedling promises enhanced ingredient penetration and skin rejuvenation, it also presents a complex regulatory puzzle, particularly when balancing the line between “cosmetic” and “medical” applications.

Originally developed in dermatology, microneedling has progressively expanded into the cosmetic sector, where it is used to improve skin appearance without being classified, in many cases, as a medical treatment.

How microneedling works

The skin’s primary function is to act as a barrier, protecting the body from external threats. Paradoxically, this same barrier—the stratum corneum—is the greatest challenge for topically applied cosmetic formulations. Most active ingredients struggle to penetrate beyond the surface layer due to their molecular weight or hydrophilicity.

Microneedling is based on a simple but effective biological principle: controlled skin damage followed by regeneration.

When the needles penetrate the skin, they create microchannels that activate a wound-healing response. This process involves the release of growth factors and the stimulation of fibroblasts, leading to new collagen formation.

At the same time, these microchannels temporarily increase skin permeability, which can enhance the penetration of topical products applied immediately after the procedure.

This dual effect, stimulation plus enhanced delivery, explains why microneedling is often combined with cosmetic formulations such as serums or ampoules.

Risks and benefits of microneedling

Benefits: From a cosmetic perspective, microneedling is used to improve overall skin quality. Reported benefits include smoother texture, reduction of fine lines, and improvement in the appearance of scars and pigmentation.

It is also considered suitable for a wide range of skin types and generally involves shorter recovery times compared to more aggressive procedures.

Risks: Common side effects include redness, irritation, and mild swelling. More significant risks may arise when the procedure is not properly controlled, including infection, post-inflammatory hyperpigmentation, and skin damage.

The risk level increases significantly with deeper needle penetration, poor hygiene conditions, or untrained use, which is particularly relevant in the context of at-home devices.

“Liquid microneedling”: how cosmetic products simulate the microneedling effect

“Liquid microneedling” is not true microneedling but rather a commercial concept used to describe topical cosmetic products that aim to replicate similar effects. These formulations may include ingredients such as spicules, exfoliating acids, or peptides designed to stimulate the skin or enhance active delivery.

However, unlike mechanical microneedling, these products do not create actual microchannels in the skin. Their effect is limited to surface-level interactions.

From a regulatory perspective, they are clearly cosmetics, but their positioning requires caution. Claims suggesting dermal penetration or structural modification of the skin could lead to regulatory challenges.

What are spicules, and where do they come from?

Most cosmetic spicules used for “liquid microneedling” are microscopic needle-like structures primarily derived from the skeleton of marine sponges or freshwater sponges, and they are typically composed of silica (SiO₂).

Although silica is the same substance found in sand and glass, in spicules it is produced biologically by the sponge through a highly controlled enzymatic process.

This natural formation results in remarkably uniform, needle-like structures with finely tapered ends. Such consistency in shape and size is difficult to replicate synthetically and is directly linked to their functional role in cosmetic formulations.

These structural characteristics enable spicules to interact efficiently with the skin’s surface, which underpins their application in skincare products.

Regulatory challenges: medical and cosmetic uses of microneedling

Microneedling is considered a “borderline product” as it sits at the intersection between medical and cosmetic applications. 

In a cosmetic context, it is mainly used for skin rejuvenation and improving overall appearance. This includes reducing visible signs of ageing, refining skin texture, and improving radiance.

In medical or dermatological terms, microneedling is used for more targeted indications, such as acne scars, stretch marks, or even as an adjunct treatment in alopecia.

This distinction is not only technical but also regulatory, as the intended use determines whether the product or device falls under cosmetic or medical legislation.

A case-by-case evaluation of all the characteristics of the product, including concentration, route of administration, frequency of application, sequence of application, application site, and the degree of penetration, considering the specific mode of delivery, should be carried out carefully to decide on the classification of the product.

Microneedling regulation: cosmetic or not?

Microneedling raises complex regulatory questions:

  • How is the microneedling product classified under applicable law in each target market?
  • Is the product intended for professional use, consumer home use, or both?
  • Are the needle length, density, and material compliant with regulations?
  • What clinical or performance data are required to demonstrate safety and efficacy?
  • Does microneedling increase active ingredient penetration beyond cosmetic limits?
  • Do labels and Instructions for Use meet regulatory language and content requirements?
  • What claims do we intend to make?
  • Do any planned claims move the product from cosmetic to medical device or medicinal product status?
  • Are “collagen stimulation” or “skin regeneration” claims acceptable in a cosmetic context?

 

In the European Union and the United Kingdom, the classification depends on the intended purpose and depth of penetration. Devices that act beyond the superficial layers of the skin are more likely to fall under the scope of the Medical Device Regulation.

In the United States, the FDA has clarified that certain microneedling devices are considered medical devices, particularly when they are intended to treat scars or affect skin structure.

This creates a clear distinction:

  • Cosmetic positioning: improving appearance without affecting physiology.
  • Medical positioning: treating conditions or modifying skin structure.

 

For companies, this distinction is critical. Claims, product design, and marketing communication must be aligned to avoid unintended classification as a medical device.

In most cases, these products are classified as cosmetics because they are applied topically and intended to improve the appearance of the skin through superficial effects. However, they can move into borderline territory or be considered as a different product category depending on how they are designed, function, and especially how they are marketed.
